Congratulations on joining the Rainbow International of Northeastern Maryland team! Your
company offers great employee benefits, including Allstate Benefits, some of which are provided
to you at no cost - yes they are free - in addition to the other benefits your company provides.
Allstate Benefits offered include:

1. Short-Term Disability Insurance: Paycheck Insurance that helps replace your income if you get
sick, injured, or pregnant and you can’t work.
2. Term Life Insurance: Pays a tax-free cash benefit to your designated beneficiary in the vent of
your death
3. Supplemental Insurance Plans:
o A supplemental Accident & Injury Plan
o A supplemental Critical Illness & Cancer Plan
o A supplemental Hospitalization Plan (Can Include Maternity Delivery Coverage).

These benefits are designed to help offset deductibles, co-pays, out-of-pocket medical
expenses, and lost income. They pay cash benefits to you, not to Doctors or Hospitals. Use the
funds however you need to. They pay in addition to any other insurance you may have and offer
optional coverage for all immediate family members.

Your company gives every employee $70/month (or $32.31 per bi-weekly pay) to
put towards any of these Allstate Benefit plans. Anything you elect over that amount, you
would pay for, and only that additional amount would be taken out of your paycheck each pay period.

You need to do 3 things:

1. Review the Allstate Benefit options in this e-Brochure Book

2. Review the customized rate sheet attached to the email this came in

3. Use the link in the e-mail to schedule a 1-on-1 phone or online video
session to ask questions, make your selections, and sign up.
