Welcome to the Allstate Benefits Electronic Flipbook.  This e-brochure will provide you
        with an initial overview of the Allstate Supplemental Employee Benefits made available
        to you through your employer. These benefits are completely voluntary, and are
        employee funded through payroll deduction. By enrolling through your company, you
        will enjoy greatly reduced group rates, and many benefit premiums qualify for pre-tax

        payroll deduction. All benefits are offered without the need for a physical exam.

        Benefits Offered include:

            • Short-Term Disability Insurance (includes Maternity Leave Benefits)
            • Term & Whole Life Insurance

            • Supplemental Insurance Plans (to help with deductibles, co-pays and out-of-
               pocket expenses):

                   o Cancer Plan

                   o Hospitalization Plan (includes Maternity delivery coverage).
