Short-Term Disability Rates





               The following 9 pages provide different option combinations for the Short-Term Disability
               Insurance. The rates are derived from several factors:

                   1. Your Age
                   2. Your Income (which determines your Monthly Benefit)
                   3. The Waiting Period selected
                   4. The Benefit Period selected
                   5. Your Payroll Mode

               Age: Your Short-Term Disability premium is calculated based on your age on the date your
               policy will be effective.  Your rate will be “locked in” thereafter, even as you age into other age
               bands. The age bands are: 18-49, 50-59, and 60-69. The older your age band, the more
               expensive the premium.

               Annual Income:  Your stable salary or your annualized hourly wage based on the number of
               hours you work per week. Find the annual income level that is closest to your annual income
               without going over.  The next column will be the Monthly Benefit amount you qualify for, based
               on your income.

               Monthly Benefit:  This is the amount Allstate Benefits would pay you, per month, if you had a
               qualifying disability. Allstate Benefits will replace a maximum of $5,000/month.
               Elimination Period: also known as the Waiting Period, is the number of calendar days you
               must be out of work before you are eligible to collect your disability payments.  Options are: 7
               Days, 14 Days, 30 Days, 90 Days, and 180 Days.  The longer the waiting period, the cheaper
               the policy premium.

               Benefit Period: The Benefit Period is the maximum number of months Allstate Benefits will pay
               you for any given covered disability. Benefit Period options include: 3 Months, 6 Months, 12
               Months and 24 months.  The shorter the Benefit Period, the cheaper the premium.
               Payroll Mode:  The rates provided have been calculated based on a Bi-Weekly payroll mode.
               This is the amount that would be deducted from your paycheck each pay period.
               We have provided rates for the following combinations:

                   •    7 Day Elimination Period with a 3 Month Benefit Period
                   •    7 Day Elimination Period with a 6 Month Benefit Period
                   •    7 Day Elimination Period with a 12 Month Benefit Period
                   •   14 Day Elimination Period with a 3 Month Benefit Period
                   •   14 Day Elimination Period with a 6 Month Benefit Period
                   •   14 Day Elimination Period with a 12 Month Benefit Period
                   •   30 Day Elimination Period with a 6 Month Benefit Period
                   •   30 Day Elimination Period with a 12 Month Benefit Period
                   •   30 Day Elimination Period with a 24 Month Benefit Period
