Welcome to the webmechanix Allstate Benefits Electronic Brochure Flipbook.  This
               e-brochure will provide you with an overview of the Allstate Supplemental Employee
               Benefits made available to you through your employer. These benefits are completely
               voluntary, and are employee funded through payroll deduction. By enrolling through
               your company, you will enjoy greatly reduced group rates, and many benefit premiums
               qualify for pre-tax payroll deduction status as part of your company’s tax-favored
               Cafeteria Benefits Plan.

               Benefits available include:

               •   Short-Term Disability Insurance – Paycheck Protection if you get sick or injured and
                   cannot work (Includes Maternity Leave Benefits)
               •   Life Insurance (up to $250,000) for yourself, your spouse, and your children
               •   Supplemental Insurance Plans (to help with lost income, deductibles, co-pays, and
                   out-of-pocket medical expenses):
                       o Supplemental Accident Plan
                       o Supplemental Cancer Plan
                       o Supplemental Critical Illness Plan

                       o Supplemental Hospital Indemnity Plan (Includes Maternity Delivery Benefits)
               Please look through the e-brochure and be sure to contact our Allstate Benefits
               Specialist, Andrew Waller, to make any benefit elections during your new hire
               enrollment window or during the annual open enrollment period.

               Many benefits are initially offered on a “Contingent Guaranteed Issue” basis (only 2
               medical questions) which makes qualification relatively easy for almost everyone. But
               these relaxed underwriting criteria are only available to you during your initial
               eligibility period.  If you decline coverage at that time, and later decide you want to
               enroll, you will have to qualify by answering more extensive medical background
               questions, the answers to which will determine if you are accepted or declined.  So,
               please make sure you look at these benefits and take advantage of the relaxed
               underwriting period if you are interested – especially if you currently have any medical
               conditions.
