Employee must buy supplemental life coverage to buy dependent life coverage.
Dependent coverage cannot exceed 50% of employee’s coverage amount.
Children eligibility: age 14 days up to age 19 or if unmarried and full time student dependent upon employee for support,
coverage can extend up to age 26.
Spouse premiums are rated on Spouse’s own age.
