Select Plan Page: Select Single/Joint Coverage. This will indicate monthly premium charge (as % of monthly utility bill) Enter information Page: Name, Address, Phone, Email, Utility Account #, Fulfillment Preference, Acceptance of T’s & C’s Billing Authorization Page: Customer authorizes UBP premium to be added to their utility bill
Confirmation Page
o Phone Enrollment
