MICHIGAN SPORTS CONCUSSION LAW
The sports concussion legislation requires all coaches, employees, volunteers, and other adults involved with a youth athletic activity to complete a concussion awareness on-line training program.
The organizing entity (CTPR) must provide educational materials on the signs/symptoms and consequences of concussions to each youth athlete and their parents/guardians and obtain a signed statement acknowledging receipt of the information for the organizing entity to keep on record.
Each of our day camp participants must turn in a signed concussion document to the Recreation Office. The office staff will let sites know which participants have not signed a form and it will be up to the sites to give copies of these forms to the participants. As a staff we will make reasonable attempts to collect these forms. A personal injury form must be filled out and returned to the office by the end of the day
The law also requires immediate removal of an athlete from physical participation in an athletic activity who is suspected of sustaining a concussion. The student athlete must then receive written clearance from an appropriate health professional before he or she can return to physical activity. This written clearance must be turned into the office before active participation can begin.

Enter your FIRST and LAST name
Watch all 5 video segments and complete the questions following each video
Print off certificate and turn it into the Recreation Office
This training with take approximately 45 minutes
Once the certificate is turned into the office, staff may add 1 hour on their payroll for completing the training
To turn in the certificate into the office, you can either take a picture of it, or take a screen shot of the certificate and email the file to the office.
