Q. What is the process to join the Golden Dental Plans network? • Providers may utilize the GDP website to digitally submit the
applications or simply contact GDP at 1-800-451-5918 to request a new provider on-boarding kit.
Q. How long does it take to join the network?
• The enrollment process takes up to 30 days, to expedite please
utilize GDP’s website, click on the provider tab and then select the join GDP network button. All enrollment forms may be digitally submitted, upon receipt of forms GDP shall deliver a contract to new provider.
