  The Risk Register
The Risk Register is a single authoritative location for the recording of comprehensive information on all known or suspected risks for an organization. It will detail the steps taken to analyze the probability of the risk showing up as well as the steps are to be taken in the event that a risk (something that might happen) turning into an actual happening. This database is used by the Knowledge Management Process to evaluate with how risk were dealt. Knowledge Management can determine if the decisions made were good decisions or bad decisions leading to the ability to alter the decision-making process in the organization. The overall goal of Knowledge Management is to create a smarter organization over time.
