Removing the Root Cause - Submission of RFC
Removal of a root-cause is a change to the operating environment and often will require approval from Change Management. When a root cause is found, a decision must be made regarding the removal. Determinations regarding cost, viability, necessity (is there a good workaround?), and effort must be considered. If it is decided to proceed, then an RFC should be submitted to change management. At this point, problem management will have done all that it can to alleviate the problem. Change management will follow their regular change management procedures

regarding priority and speed. During this time, incidents may continue to occur. Problem management will put the problem record in a status of “resolved”, but not closed while waiting on Change management to approve and Release and Deployment to perform the change. This status signals all involved that the change management process is now in charge. “Don’t call and yell at problem management. We are waiting on Change management!”
