In-Person Survey Reminders
• The survey is
• Voluntary. People can choose not to do the survey. They can also choose to skip
questions they are not comfortable answering
• Confidential. Names of participants, case managers, staff, providers, etc. are not tied
to the survey; you should not share information from the meeting with anyone other
than your NCI coordinator (with expectations for following mandated reporting or
unmet need follow up). Remember, this survey is not intended to make direct
changes to the services people receive.
• About one hour. You should hold about and hour and a half to two hours for each
survey to allow for any technical support or other issues you may encounter. People
can take breaks as needed and you may stop and restart remote surveys at a later
date if needed.
• DO NOT SCHEDULE TOO MANY SURVEYS IN A DAY. Surveyors should aim to
complete 2-3 per day, no more than 4.
