Practice Survey Instructions

• Surveyors will be broken out into groups of 2-3 people

• Surveyors will need the In-Person Survey available (either on the computer as a
PDF, using a demo ODESA account, or on paper)

• In break-out groups:

• One person starts as the surveyor, the other person will play role of person being surveyed

(groups of three will have one person following along in the survey and observing)

• “Surveyor” should ask questions in one of the following sections:

• Employment/Other Daily Activities – pg. 39

• Satisfaction With Services/Supports – pg. 51
• Community Inclusion – pg. 59

• COVID Supplement
• After 5-7mn, the person surveyed (and observer if appliable) will provide feedback to the

surveyor

• Switch roles
