5.       Fund Sourcing and Requisition -The           prepared      proposals      were
               presented,  evaluated,  screened  and  funded.  All  General  Appropriations  Act  (GAA)  –
               General Funded extension and training programs of the            BSHM Program fall on the
               identified priorities - Improvement of instruction, technology generation and human and
               natural resources development while the students’ E&T proposals are funded by Student
               Trust Fund (STF). In cases of limited funds, the faculty and students are allowed to source
               out funds in the form of counter parting.
                     For  requested  trainings,  the  College  counterpart  is  on  the  area  of  technical
               assistance as they (Faculty members and students) act as the trainer, co-trainer and
               lecturer. The requesting agency/association/team/group will send a Letter of Request to
               the  University  President  or  Campus  Executive  Director  for  approval  mentioning  the
               responsibilities of both parties particularly on cost-sharing part.
                       6.     Training Implementation – During the training the BSHM students do the

               following – preparation of communication letters, program invitation, attendance sheets,
               venue and fixing the materials needed. They serve as extension/training facilitators during

               the actual conduct of the training activity. Faculty in-charge, College E&T Coordinator
               accompanies  the  students  during  the  actual  extension  of  trainings.  They  serve  as

               evaluators  of  students’  performances  for  the  conducted  extension  activity  as  partial
               requirement to the subjects they enrolled with skills performance tests.
                       7.     Training Evaluation – Evaluation forms were given to the trainees after

               training. This is one way to determine the strengths, weaknesses and limitations of the

               conducted training for improvement using the DET Forms. Trainer and the students are
               required to consolidate the results of evaluation as part of the accomplishment report/
               terminal report which will soon be submitted to the Campus E&T Chairperson and to DET.

                       8.     Report Preparation- College Extension and Training Coordinator and the
               students are expected to submit accomplishment report to Campus E&T Chairperson
               following the format given by DET as to preparation of terminal report which includes:
                       Summary (an Article for the annual publication -E&T in the Spotlight); Program;
                       Attendance sheet;
                       Approved training proposal;
                       Lecture Notes/Powerpoint Presentation;
                       Evaluation forms with Summary of Evaluation;
                       Photo documents and appending the sample training certificates.
