TEXAS MUTUAL INSURANCE COMPANY VISION PLAN
Plan Year – a period of time beginning with the Plan anniversary date of any year and
terminating exactly one year later. If the Plan anniversary date is February 29, such date will
be considered to be February 28 in any year having no such date.
Provider – any optometrist, ophthalmologist, optician or other person who is properly
licensed and qualified by law to provide Services.
Services – any covered benefit listed in Section 5, Additional Coverage Details.
Spouse – an individual to whom you are legally married.
