DIABETES MELLITUS
Blood pressure determination, including orthostatic measurements
when indicated
Fundoscopic examination
Thyroid palpation
Skin examination (e.g., for insulin injection or infusion set insertion
sites)
Comprehensive foot examination
o Inspection
o Palpation of dorsalis pedis and posterior tibial pulses
o Presence/absence of patellar and Achilles reflexes
o Determination of proprioception, vibration, and
monofilament sensation .
C. Diagnostic studies, including:
A1C
Fasting lipid profile, including total, LDL, and HDL cholesterol
and triglycerides, as needed
Liver function tests
Spot urinary albumin–to–creatinine ratio
Serum creatinine and estimated glomerular filtration rate
Thyroid-stimulating hormone in patients with type 1 diabetes or
dyslipidemia or women aged > 50 years
Referral, if feasible, to:
Diabetes educator, to evaluate patient’s ability to perform self-
monitoring of blood glucose and his/her ability to interpret the
data
Registered dietitian for MNT
Foot-care specialist
Ophthalmologist for annual retinal screening, or more often as
indicated
Nephrologists, neurologist, and cardiologist, if needed.
Family planning for women of reproductive age
Dentist for comprehensive dental and periodontal examination
Mental health professional, if indicated
