Imagine Nation requires individual care plans for medical conditions such as allergies,

         asthma, seizures, diabetes and other health conditions that may require some type of
         accommodation. These care plans must be updated by the child’s physician each year or if



         Chapter 36 of the Health and Safety code requires all children enrolled for the first time in
         any public, private, parochial or denominational school or in a Department of Family and
         Protective Services licensed child care center and child care home in Texas, or who meet
         certain grade criteria must be screened or have a professional examination for possible
         hearing and vision problems. Children who are 4 years old by September 1 of the current




         When to Keep Your Child at Home
      ◆  year must be screened within 120 days of admission.
         For the health and well-being of your child and others, if a child develops any of the
         following contagious symptoms, they must be kept home until symptom free for 24 hours
         or until the child’s physician indicates the child can return to the school:
         •  Uncontrolled diarrhea
         •  Severe Coughing
         •  Difficult or rapid breathing
         •  Yellow skin or eyes
         •  Tears, redness of eyelids with discharge
         •  A fever of 100 degrees or above
         •  Unusual spots or rashes
         •  Sore throat or trouble swallowing
         •  Infected skin patches
         •  Severe itching of body or scalp
         •  Vomiting
         •  Infestation or parasite (ex. Head lice, scabies, pin worms)
         •  Any other contagious or communicable disease


         In certain cases, it may become necessary to require a physician’s written release stating
         the child is no longer contagious.
