• True splenic cyst have thin walls, an anechoic center, and posterior enhancement.
• However,cystsfoundinthespleencanappear complex, particularly those associated with trauma.
• Other complex cysts could be displaced pancreatic pseudocysts, congenital cysts, hydatid cysts, or cystic metastasis.
