Proof of Identity
• Proof of identification must be issued by the United States government (Federal or State)
• Out-of-state identification is only acceptable if Proof of Eligibility is in-state
List of Acceptable Forms of Identification:
• U.S. Driver’s License
• U.S. Government, Military, State, or Tribal Issued ID
• Weapons Permit
• United States Passport
• United States Permanent Resident Card
• United States Permanent Resident Alien Card
• Birth Certificate
• Social Security Card
• Certificate of U.S. Citizenship
• U.S. Certificate of Naturalization
• Military Discharge Documentation
* The following are Acceptable Forms of Identification that also serve as program eligibility
documents. Examples can be found in the Proof of Eligibility Samples Document.
• Government Assistance Program Document (Must include either date of birth
or last four digits of social security number)
• Statement of Benefits from a Qualified Program (Must include either date of
birth or last four digits of social security number)
• Unemployment/Workers compensation statement of benefits
• W-2
• Prior Year’s State, Federal, or Tribal Tax Return
List of Unacceptable Forms of Identification:
• Mexican/Matricula ID
• Social Security Card
• Non-US Issued Passport
• Non-US ID
• School Issued ID
• Employee ID
