Unacceptable Proofs of Eligibility

SNAP/Food Stamps Card:

The following Iowa SNAP card is not accepted because there is no beneficiary name listed on
the card:

Medicare Card:

Supplemental Security Income (SSI) Program:
The following are NOT acceptable as proof of participation in the SSI Program:
o Any document that shows the applicant is receiving Social Security benefits but
not SSI benefits. SSI benefits are administered through the Social Security
Administration but should specifically state that the applicant is enrolled in and
currently receiving benefits through the SSI program.
