Unacceptable Proofs of Eligibility
Minnesota Food Assistance Program (Food Stamp) Card:
*Minnesota SNAP cards are not accepted because they do not have the cardholder’s name
printed. A customer can provide a printout or screenshot from an online portal or website tool
with the program name, consumer’s name, case/identification number, eligibility dates, and date
site accessed.
Medicare Card:
Supplemental Security Income (SSI) Program:
The following are NOT acceptable as proof of participation in the SSI Program:
o Any document that shows the applicant is receiving Social Security benefits but
not SSI benefits. SSI benefits are administered through the Social Security
Administration but should specifically state that the applicant is enrolled in and
currently receiving benefits through the SSI program.
o Any document showing that the customer has applied for SSI benefits and that
the application has been accepted but not showing that the benefits have been
approved or eligibility certified.
