 5. First Aid Measures – (taken from COSHH sheets) Tells you what First Aid should be administered for each of the products chosen for the project. You should take out this information out of the Plan and place it with your site first aid kit -making it immediately available in an emergency.
6. Lifting Appliances Inspection – For recording the regular inspections you need to do, guidance is provided in the sheet.
7. Scaffold & Excavation Inspection – For recording the necessary inspections. Guidance is given in the sheet.
Section Five - Permits to Work
1. Hot Works Permit - To be used for all hot works. A separate permit each time hot works are carried out. This provides you with (and demonstrates) additional management control over hot works. You and the person carrying out the works must complete it.
2. Roof Work Permit – To be used for all roof works. This provides you with (and demonstrates) additional management control over roof works. You and the person carrying out the works must complete it.
3. Electrical Work Permit – To be used for any work where there is a risk of a person coming into contact with live electrical services. This provides you with (and demonstrates) additional management control for the electrical safety on site. You and the person carrying out the works must complete it.
4. Confined Space Work Permit – To be used for any work taking place in a confined space. This provides you with (and demonstrates) additional management control over such work. You and the person carrying out the works must complete it.
5. Excavations Work Permit – To be used for work digging or taking place in excavations. This provides you with (and demonstrates) additional management control. You and the person carrying out the works must complete it.
Section Six - Statutory Records
1. Accident Book BI 510 – To record accidents.
2. Notice of Accident – Guidance is given on the reporting of accidents, the F2508 is provided for the
reporting of accidents to the HSE under RIDDOR.
3. Site Attendance Record/Visitors Book – To record personnel visiting or working on site. This is particularly important should the site need to be evacuated in an emergency. It is important to sign people off site as well as onto site.
