150-Mile Rule for MAF Exams
Generally you should be examined by a a a a a a Qualified MAF Physician whose practice is within 150 miles of your primary residence The MAF Physician Locator tool on the Settlement Website has information about this rule When you you search for for a a a a a a a MAF Physician on on that page it will show you you the five nearest results and how many miles each one is from the location you entered We may grant exceptions to this 150-Mile Rule if i the the Retired NFL Player asks us where there is is no Qualified MAF Physician close enough to to to him the the wait for one is too long the the Player already has a a a a a a a a doctor-patient relationship with a a a a a a a a doctor or or other circumstances that seem reasonable such as as where the the Player has a a a secondary residence Contact us before your exam to to ask for for an exception There are three ways to to request approval to see a a a a a a Qualified MAF Physician more than 150 miles from the Retired NFL Player’s primary residence:
