2) A stand-alone drug plan. To start with, please verify that the drugs, dosages, and
monthly quantities that I entered for your two drugs are correct. They are shown in
Appendix B1. If changes need to be made, let me know and I’ll make the
adjustments and see if it changes the ranking of the plans.

Appendix B2 is a ranking of the stand-alone drug plans by their costs for the drugs
and dosages in Appendix B1. In this ranking, the SilverScript SmartRx plan is the
lowest-cost stand-alone plan for your drugs.

You can enroll in this plan by calling (833) 526-2445. Its benefit summary is in
Appendix B3. CVS is a preferred pharmacy in this plan, and your pharmacy can
probably transfer your current prescriptions in this plan.

Sometime later this week or early next week, I’ll send you a more complete
explanation of your Medicare coverage options and things you need to be aware of.
Cost for your evaluation will be $175, and I’ll send you an invoice for in a few weeks.
