               Dear Sue:

               Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
               page 5 to make sure it is correct. The costs shown in your evaluation are estimates for the
               entire 2019 plan year, although you will be enrolled in Medicare for only 10 months this year.

               As I mentioned in my e-mail, you can call Social Security at (800) 772-1213 to enroll in Part A
               and Part B (you can also enroll online at ssa.gov or by scheduling an appointment at your local
               Social Security office). Within 2-3 weeks after you enroll, you will receive your Medicare card in
               the mail indicating your Part A and Part B coverage will start on March 1.

               Your supplemental coverage options

               Once you’ve signed up for Parts A & B, your next step is to choose your supplemental coverage
               and have it in place by March 1. When you enroll in your supplemental coverage, it will include
               your Part D benefits. If you choose a Medicare Advantage plan, for instance, it will include your
               Part D coverage as part of the overall benefit package. And if you get a Medigap policy, you will
               need to enroll in a Part D stand-alone plan.

               After you’ve reviewed the evaluation and decided on the type of coverage that you want, you
               can follow these steps to enroll the plan(s) that you’ve chosen:

                            If you decide to enroll in an Advantage plan, you can call the plan’s toll-free
                              number shown on pages 2-3 of the evaluation. Make sure that the person you
                              speak with understands that you want your coverage to begin on March 1.

                            If you decide to get a Medigap policy and a Part D stand-alone plan, you should
                              first decide which Medigap plan you want – Plan G or N or another plan. Then
                              we suggest that you call three or more insurers before deciding which company
                              you’ll get your policy from.

                            Finally, you can enroll in the Part D stand-alone plan that you choose by calling
                              the plan’s toll-free number that’s shown on page 4 of the evaluation.
