               Dear Ms. Weade:

               Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
               page 5 to make sure it is correct. Also, please verify that your prescription drug information
               shown on the first page of Appendix C1 is accurate.

               The estimated costs in your evaluation are for the 2019 plan year. And as you know, if you
               decide to change your coverage you will need to do so before the end of Medicare’s fall open
               enrollment period on December 7.  If you do change plans, here are the steps to follow:

                       1)  To switch to a different Medicare Advantage plan, call the plan’s toll-free number
                          that’s shown on the evaluation. Be sure to specify the exact name of the plan –
                          there are several Humana plans, for instance, and it’s important to make sure you
                          are switching to the one you want.

                          You do not need to dis-enroll from your current plan. When Medicare is notified that
                          you have enrolled in another Advantage plan for 2019, it will dis-enroll you from
                          your current plan at the end of the year.

                       2)  To switch to a Medigap policy, you need to take two steps: a) buy a Medigap policy
                          and b) enroll in a Part D stand-alone plan. When Medicare is notified that you have
                          enrolled in a Part D stand-alone plan, it will dis-enroll you from your Advantage plan
                          at the end of the year.

                          It’s a good idea to get the Medigap policy first so that you know what your premium
                          will be – as opposed to getting the stand-alone drug plan and then finding out it may
                          be difficult to get an affordable Medigap policy.

               Two of your providers accept Medicare-approved rates

               Drs. Garrean and Mummert are listed in the Medicare provider directory and accept Medicare-
               approved rates. It’s likely that some of Dr. Mummert’s services are covered by Medicare while
               others are not. Medicare does not cover routine vision care but it does cover services to detect
               and treat eye diseases like glaucoma, cataracts, and macular degeneration. Drs. Teel and
               Gabrek are not listed, which is not unusual because Medicare does not cover most types of
