Besides these two plans, there are two others that may be good candidates for you. One is the
Humana Choice PPO plan, which also lists Drs. Garrean and Mummert in its provider directory.
But this plan has a $6,700 out-of-pocket limit, which is not unusual for an Advantage PPO plan
but perhaps more risk than you would want to take in the event of a serious illness or expensive
treatment. Also, both the HMO plans in your evaluation have large provider networks, which
make it less likely that you would benefit from the PPO’s out-of-network coverage.

Another plan that might be a good choice is the Indiana University Health Plans Medicare Select
HMO plan. In your case, this plan’s most appealing feature is that if you are willing to switch to
mail-order refills, you will have zero costs for your prescription drugs in 2019. This plan has a
3.5-star quality rating from Medicare, but Dr. Mummert is not listed in its provider directory. If
this is a plan you are interested in finding out more about, I can send you its benefit summary,
and there’s also a slight chance that Dr. Mummert is in this plan’s network but is not listed. You
can verify whether that’s the case by calling her office.

               Rx Drug Plan Coverage

Your drug costs are shown on page 4 of your evaluation. If you purchase a Medigap policy, you
will also need to enroll in a Part D stand-alone plan. The lowest-cost stand-alone plan for your
drugs next year is Humana Preferred Rx Plan, and its estimated costs next year is $826 if you
get monthly refills at a CVS pharmacy.

If you remain in your current plan, your 2019 costs are an estimated $760 if you get refills at a
CVS Pharmacy. And you can trim those annual costs by more than $260 if you switch to mail-
order refills. You will need new prescriptions from your doctors if you change from monthly to
               mail-order refills.

The Humana Gold Plus Plan has the lowest annual Rx drug costs of the plans compared in your
evaluation -- $496 if you get monthly refills at CVS and $319 if you switch to mail-order refills.
And as noted above, if you switch to the IU Health Plans Medicare Select HMO, your mail-order
costs next year will drop to zero.

Ms. Weade, please let me know if you have any questions.
