Specialized Care: If you are enrolled in a plan that requires referrals, you will need a referral
in order to use your benefit most effectively. When you need specialist care, your primary care
physician will refer you to a network specialist or hospital he or she routinely uses. Many primary
care physicians limit their referrals to selected specialists and hospitals so be sure to check
with your physician whenever you need a referral. You should not assume that the services of a
particular specialist or hospital listed here will be covered according to your benefits, unless
you have the appropriate referral.
State and federal regulation, along with plan option selection, may allow for a broader range of
provider access. Members should consult their plan documents.
Copayment amounts for participating providers may vary depending upon the benefit plan you have
chosen. Please check your Certificate of Coverage or Plan Benefit Document to see if your plan has
different provider copayment levels. You may also contact Humana Customer Service for the
appropriate copayment schedule by calling the number on your ID card.
