 Faith in Action Initiatives
Marking Milestones
Faith in Action Initiatives (FIAI)
speaks to the tremendous value
BSWH places on holistic care, as
well as its commitment to
bettering the lives of staff and
communities both close to home
and thousands of miles away. FIAI
achieves this through three major
programs – Faith Community
Health (FCH), Sacred Vocation and Second Life Resource Centers, as well as by assisting staff in personal efforts to make a difference.
Collaborating to Help Be Better
FCH seeks to leverage faith communities
to improve community health. For
example, FCH coordinator Carlos Valencia
hosted a spring health fair at a Fort Worth church, where 130 participants took advantage of health screenings and attended workshops on financial, physical, emotional and mental health. One congregant was referred to Baylor Scott & White Community Care Clinic — Fort Worth after an elevated blood glucose reading at the church’s monthly health screening. She received treatment and education resulting in lifestyle changes to control blood sugar levels.
FCH also launched a new initiative for frequent Emergency Department (ED) patients at BUMC and Baylor Scott & White
— Hillcrest, offering to pair these patients with a Faith Community Caregiver (FCC) volunteer. The FCC volunteer gives one hour each week to accommodate the needs of the patient. The first 50 patients at Baylor Scott & White — Hillcrest have reduced their ED utilization by 35 percent.

162 Patient referrals FCH made in FY2018.
