Monthly Cost of Coverage

You pay a monthly premium for each $1,000 of coverage. Your premium for
yourself is based on your age at time of employment and will not increase until the
next enrollment period regardless of having a birthday which takes you to the next
age band.


The following age reduction schedule applies to basic employee and dependent life
insurance as well as voluntary employee and spouse life insurance and voluntary
employee and spouse AD&D coverage.

X 65 percent of original amount at age 70

X 50 percent of original amount at age 75

For coverage to be effective for yourself, your spouse, and/or
dependent children, you must be actively at work on the day your
coverage is effective. Your spouse and dependent child(ren) must be

performing the task of a similarly situated spouse (children). If not,
coverage will not be effective until you return to active employment or
your spouse and/or children are performing normal duties.
