Group Life Insurance
Life insurance is provided through The Standard Insurance
Company.
Basic Group Life Insurance
The University of Southern Indiana provides you a life insurance
beneit which is a multiple of your salary up to $100,000, at no cost
to you. If you were hired after February 1, 1988, you are provided
life insurance of 1.5 times your salary up to $100,000.
Basic Accidental Death and Dismemberment
(AD&D) Insurance
If your death is the result of an accident, you will receive a beneit
for the same amount of the basic life beneit. The plan will also pay
a beneit amount if an accident leaves you with certain debilitating
injuries.
Designating Your Beneficiary
If you die while covered by this plan, the beneit is paid to the
beneiciary you designate during enrollment. Please make sure
your beneiciary information is accurate. You can change your
beneiciary information outside of enrollment at any time.
