Your 2016–17 Beneits Guide
Welcome to the Optimas OE Solution Holdings, LLC(Optimas) beneits
program. As an employee, you play a tremendous part in our company’s
success. This is why a core tenant of our business strategy is to ensure
you are offered an attractive and competitive beneits package. This
beneits guide provides detailed information about all of the health and
welfare beneits offered to you. This is a summary of beneits. Detailed
eligibility and beneit descriptions are available on ADP/WFN portal, or
by contacting Human Resources.
Now is your opportunity to enroll in coverage for you and your eligible
dependents. You will not be able to change your medical, dental, vision,
voluntary accident, and lexible spending account elections during the
middle of the plan year unless you experience a qualiied life status
change. Qualiied life status change events include, but are not limited to
the following.
Marriage
Divorce
Birth or adoption of a child
Changes in your or your dependents’ employment or coverage status
Death
It is your responsibility to log into ADP/WFN within 31 days of the
event to request the change. Failure to do so within 31 days will prohibit
you from changing your beneit election(s) until the next annual open
enrollment period.
Please note: you may elect to increase, decrease, or terminate your HSA
contributions if enrolled in the consumer HSA plan as well as your and
your dependent’s supplemental life coverage at any time throughout the
year. You must log into the ADP/WFN portal to request this change.
Evidence of insurability and approval from Reliance Standard Life may be
required in certain circumstances.
You may adjust your commuter transit and parking contributions any time
throughout the year as well by logging into the ADP/WFN portal and
requesting the change.
