To determine the monthly premium for voluntary life coverage, follow
these simple steps:

X Determine the amount of insurance you want to purchase

X Using the table, determine the rate based on your age

X Use the formula following to calculate your monthly premium
amount

Coverage Amount/1,000 x Rate = Monthly Premium

Short Term Disability Insurance Coverage

Short Term Disability (STD) protects your income by providing you
with a percentage of your income while you are unable to work due

to an accident or sickness. The beneit is equal to 60% of your weekly
salary, up to a maximum of $1,000 per week and is offered through
Mutual of Omaha. Beneits would begin on the 8th day of an accident
or sickness and may continue for up to 13 weeks. Olshan Properties
provides this beneit at no cost to you.
