Voluntary Life Insurance
Americare offers you the ability to purchase voluntary life insurance through Guardian at competitive group rates.
Voluntary Life Insurance can be purchased in increments of $25,000 up to $150,000 for employees ($50,000
for spouse and $10,000 for children). This plan is portable; if you change jobs or retire, you can keep your life
insurance.
This year only, coverage is guaranteed issue; no medical questions. The guaranteed issue period applies to
this Open Enrollment and when you are irst hired only. If you do not enroll during your initial eligibility period,
you will be required to complete medical questions in the form of an “evidence of insurability” form.
Premiums and Benefits
X Spouse coverage premiums are based on the Beneits reduce to:
employee’s age; coverage for spouse terminates at X 65% of your original beneit at age 65
spouse’s age 70
X Premiums for voluntary life insurance increase in X 40% of your original beneit at age 70
ive-year increments X 25% of your original beneit at age 75
X 15% of your original beneit at age 80
Rates are based on your age and beneit amount elected and will be shown during your online enrollment.
