submit a written request to your respective AIP Aerospace Human Resources

written request. You may not be denied access to services provided under AIP
Aerospace for requesting AIP Aerospace to cease the use of your social security
number. AIP Aerospace will be authorized to collect, use or release your social
security number as required under existing or amended state or federal laws, for
internal veriication or for administrative purposes.
Patient Protection Disclosure
HMO medical plans generally require the designation of a primary care provider.
You have the right to designate any primary care provider who participates in their
network and who is available to accept you or your family members. Until you
make this designation, your HMO carrier may designate one for you.
For children, you may designate a pediatrician as the primary care provider.
