What is a Preventive Service?

A preventive service might be a test, or it might be advice from your
doctor. Preventive services can detect disease or help prevent illness

or other health problems. Below are a few of the common preventive
health services the plan covers.

Covered Services for Children and Adolescents
„ Well child exams—physical exams and vision acuity

„ Assessments—developmental and behavioral
„ Immunizations—diphtheria, tetanus, and pertussis

„ Screenings—hearing loss, lead poisoning, and depression

Covered Services for Adults

„ Annual preventive care visits—physicals and history
„ Immunizations—hepatitis and inluenza

„ General health screenings—blood pressure, cholesterol, and
diabetes

„ Prescription contraceptives for women
ˆ Generic contraceptives—no copay

ˆ Brand name contraceptives—$50 copay
