Vision

Greenville Regional Hospital also offers you the choice of two vision plans. The Core Plan is provided to you at no
cost. You also have the choice of “buying up” to the Buy-Up Plan which offers richer beneits.

Basic Life Insurance

All full-time and part-time beneit eligible employees will receive a basic life beneit of 1x annual salary. Spouses of
employees will continue to receive a $5,000 beneit, and dependent children (age 6 months to age 19) will continue to
receive a $2,500 beneit. Dependent children under the age of 6 months will receive a $100 beneit.

Optional Life Insurance/Optional Accidental Death & Dismemberment Insurance


Greenville Regional Hospital partners with Prudential to offer an optional life insurance plan and optional AD&D
plan to all employees. You have the option to increase your election by $10,000 increments to an additional $40,000,
not to exceed the plan maximum of 5x your annual earnings or $500,000 without answering medical questions. See
Prudential for an enrollment form. Late enrollees will need to answer medical questions for all amounts elected. Please
contact Human Resources if you have any questions regarding this great beneit!


Short Term Disability

Full-time and part-time beneit eligible employees have the opportunity to participate in a voluntary short-term
disability plan. Two options are available. Option 1 is a 40 percent beneit up to $500 per week. Option 2 is a 60
percent beneit up to $2,000 per week. If you are enrolling in STD for the irst time, you will need to submit EOI to
Prudential. Let Human Resources know if you have any questions.
