2016 Beneits Guide
Voluntary Spouse Life
You can elect Voluntary Life Insurance coverage for your spouse
if you have choosen voluntary life for yourself as well. You can
purchase coverage in increments of $15,000, $25,000, or $50,000.
Your spouse’s coverage cannot exceed $50,000 and you must elect
coverage on yourself before you can elect spousal coverage. The
cost per each $1,000 of coverage is the same as the employee cost.
Your current Voluntary Spouse Life coverage will rollover to next
year unless you make changes during Open Enrollment. If you’re
enrolling within 30 days from your hire date, you are automatically
approved. If you’re enrolling outside of this period, or increasing
the level of coverage, you will be required to submit an evidence of
insurability application subject to approval from CIGNA. Beneits
reduce to 65 percent of the original beneit at age 65, 40 percent at
age 70, and 25 percent at age 75.
Voluntary Dependent Life
Dependent child life coverage is available if you have choosen
voluntary life for yourself as well. You can select increments of
$10,000 or $20,000. Dependents are eligible from 14 days old
through age 26. The cost for dependent child coverage is $0.10 per
$1,000 of beneit. For example, if you elect $10,000 in voluntary
child life coverage it will cost $1.00 per month, regardless of the
numbers of children you are insuring. Your current Voluntary
Dependent Life coverage will rollover to next year unless you make
changes during Open Enrollment. No EOI form is required for
dependent life enrollment/increase.
Employee Assistance Program
As an ACPI employee, you are able to take advantage of our
employee assistance program with Cigna. The EAP program offers
you face-to-face counseling sessions in the event of traumatic life
events as well as on-site critical incident stress debrieing. You can
also receive free legal and inancial consultations to discuss your
personal inances.
