Eligibility
Annual Open Enrollment
It is time for your annual beneits enrollment for the 2016 plan year.
Open enrollment kicks off on Monday, November 16, 2015 and ends
on Wednesday, November 25, 2015.
Please note you will not be able to make changes to your coverage
again until the next annual enrollment unless you have a qualiied
change in status.
All enrollment forms must be completed and returned to Human
Benefit Offerings for Resources by November 25, 2015 to ensure coverage for the 2016
2016 plan year.

Vision
Life and accidental death & You are eligible for beneits if you are a regular, full-time employee,
dismemberment and work at least 30 hours per week. Your dependents who are

Short term disability

Voluntary accident
Up to age 26, regardless of student or marital status
Voluntary critical illness
