Additional Women’s Preventive Health Services
Medical Plan Women’s preventive health services, such as routine mammograms,
cervical cancer screenings, and prenatal care, are covered at 100% with
Features no charge against your plan deductible or coinsurance expenses. Note,
preventive care services are only covered in-network. Frequency limits,
medical history, or age requirements may apply for certain services
to be considered preventive care. Contraceptive medications will be
covered at 100% without a copay, including sterilization and barrier
methods such as diaphragms. For speciic preventive coverage contact

Disease Management
ASARCO has partnered with UMR to provide a Disease Management
program. Through the guidance of this program, our insured
members who suffer from chronic conditions such as asthma, chronic
obstructive pulmonary disease (COPD), congestive heart failure,
coronary artery disease, depression, diabetes, and hypertension are
offered special assistance in monitoring their condition to improve
their health. If you suffer from one of these conditions, you may be
identiied by UMR and invited to participate in the disease management
program.
The program is completely conidential and your “protected
health information” (PHI) will not be shared with ASARCO. Your
participation is optional, and is designed to help you better manage
your health conditions and improve the quality of your life.
