Tobacco-Free Discount
In an efort to encourage healthy lifestyle choices, employees who do not use tobacco/nicotine will receive
discounted medical rates. The discount is $11.54 per pay period.
You will attest to your tobacco status during the enrollment process. Tobacco cessation programs are
available to help you quit.
Deinition of Tobacco/Nicotine Use: An employee who uses tobacco/nicotine products which includes any
product that contains tobacco or delivers nicotine.



Voluntary AD&D
If you elect Voluntary Life coverage, you will also receive Voluntary AD&D coverage of the same
beneit amount.



Short Term Disability
Our Short Term Disability coverage is changing with MetLife. If you elect coverage, your beneit amount
will be 60% of your earnings to ensure you have adequate disability protection. With this coverage
enhancement, the cost of coverage is increasing.
One Time 2022 Open Enrollment Opportunity!
If you elect coverage for 2022, evidence of insurability or medical underwriting will not be required. If you
do not elect coverage during this open enrollment and elect coverage in the future, medical underwriting
will apply and is subject to carrier approval.

November 22 through December 2

Complete your enrollment at SummitBHC—EMB Beneits.


Summit BHC is holding ACTIVE enrollment. This means you must make elections to ensure
you have coverage in 2022.


After open enrollment closes on December 2, you will not be able to make any changes until the next open
enrollment period unless you have a qualifying life event.
A complete listing of all beneits with detailed plan coverage will be included in the Beneits Guide posted in
the enrollment portal at SummitBHC—EMB Beneits.
