Beneits Enrollment
You have 30 days from date of hire to enroll in your beneit elections to ensure they begin
on time. If you miss this new hire window to enroll, you must wait until Open Enrollment or a
Qualiied Event to enroll.
Please read this guide to learn more about your beneits.
During enrollment you have the opportunity to:
Add coverage for yourself
Add dependents to coverage
Any new dependents will be subject to an audit in order for coverage to remain in
place; you will receive a letter from our auditors with the appropriate documentation
and deadline to provide
Qualifying Events
A qualifying event is a change in your personal life that may impact your eligibility or
dependent’s eligibility for beneits.
Examples of some qualifying events include:
Change of legal marital status (i.e., marriage, divorce, death of spouse)
Change in number of dependents (i.e., birth, adoption, death of dependent, ineligibility
due to age)
Change in employment or job status
It is your responsibility to contact the Employee Service Center at 866.213.8919 within 30
days of the qualifying life event. Failure to notify the Employee Service Center within 30 days
will prohibit your ability to change your beneits election(s).
