Annual Enrollment
It’s time to make some important decisions about beneits for the
2015 plan year. Annual enrollment will take place from November 10
through November 28. You must make an election during this period
to have or continue to have coverage in 2015. Please review this guide
to learn more about your beneits. All elections placed during this
annual enrollment period take effect January 1, 2015.
How to Enroll for 2015 Benefits
Follow the instructions in the iSolved Employee Self-Service Booklet
to enroll in beneits for 2015.
Eligibility
You may enroll in the beneits program if you are a regular full-time
employee who is actively working a minimum of 30 hours per week.
Your eligible dependents include:
X Legal spouse, who does not have access to his/her own Employer-
Sponsored Health Plan
X Dependent children up to age 26
X Legal wards; children for whom you are a legal guardian, up to
age 26
