Associate Choice Plan

All full-time employees working 20 hours or more per week are

eligible to purchase the Associate Choice Plan. MotoMart offers
two plans administered through Bay Bridge Administrators, a
partner of Humana. These plans are designed to provide you with
affordable options which provide a range of beneits and help
satisfy the individual mandate required under the Affordable Care
Act (ACA) also known as Obamacare. By agreeing to participate
in either Associate Choice Plan, you will not have to pay a penalty

to the government for not having health coverage. For 2016, these
penalties will increase to the higher of 2.5 percent of your annual
household income or $695 per person (plus $347.50 per child over
age 18) in your household.

Both of the plan options include preventive care beneits which
cover 100 percent of your preventive service charges when
performed by an in-network provider. In addition to preventive
coverage, the Associate Choice Plan includes the following types
of beneits. The level of beneits depends on which plan you
elect.

„ Daily hospitalization „ Critical illness
„ Surgical beneits „ 24-hour telemedicine

„ Emergency room „ Prescription medications

„ Physician ofice visits
