Basic Life and Accident Insurance
Optimas is proud to provide, at no cost to you, basic life with accidental
death and dismemberment insurance. This coverage offers inancial
protection to you if you suffer certain debilitating injuries or to your
beneiciaries if you die. Your coverage amount is equal to 1.5 times your
covered annual earnings up to$250,000. Your life insurance beneit also
includes accidental death and dismemberment beneits. As you enroll
for your 2015–2016 beneits through the ADP/WFN portal, you MUST
designate your life insurance beneiciary.
Supplemental Life Insurance
You may also purchase supplemental life insurance at competitive group
rates. You may purchase coverage in increments of $5,000 to a maximum
of 5x your annual salary or $500,000 (whichever is less). Evidence of
Insurability (EOI) or proof of good health will not be required under the
following circumstances.
Newly eligible: you may elect up to $300,000 of coverage without
EOI during this initial new hire enrollment opportunity
If you request coverage in excess of the amount noted above, you must
complete an EOI form and be approved by Reliance Standard Life.
Your individual premium rate will depend on your age and will be paid for
on an after-tax basis. View the chart on the next page to determine how
much this beneit will cost. As you enroll on the ADP/WFN portal, the
cost of your election will be calculated for you.
You must be actively at work on your 61st date of employment for your
beneits to be effective. If not, coverage will be effective on the irst day
you return to active employment.
