Benefit Basics

Eligibility for Benefits

All full-time employees are eligible for medical, lexible spending accounts (FSA),
dental, and vision benefits on the first of the month following 60 consecutive days
of employment.


Dependent Eligibility
Medical and FSA Coverage
Eligible dependents include your spouse, children up to age 26, and your disabled

children of any age as long as disabled before 26.

Vision and Dental
Your unmarried children, stepchildren, legally adopted children; or any other children

related to You by blood or marriage who:

X Live with you in a regular parent-child relationship; and/or

X You claimed as a dependent on your last filed federal income tax return; or
provided such children are primarily dependent upon you for ½ of their
financial support and maintenance and are :

Z Under age 26; or
Z Age 26 or older and disabled; such children must have become disabled
before attaining age 26; you must submit proof, satisfactory to us, of such
children’s disability.


Decline Coverage

If you decline coverage under this plan for yourself or your dependents and
experience a qualifying event, in the future you may be able to enroll yourself or
your dependents in this plan provided you request enrollment within 30 days after
your other coverage ends. Examples of qualifying events are marriage, death, birth,
adoption, or loss of medical coverage.
