Special New Hire Opportunity!

During your initial new hire eligibility period, you may elect optional life coverage up
to the guaranteed issue amount without submitting evidence of insurability. Going
forward, if you want to increase your coverage amount, evidence of insurability will
be required before AIG will inalize and approve your election. The guaranteed issue
amounts are as follows.

X Employee guaranteed issue amount = $200,000

X Spouse guaranteed issue amount = $30,000

X Child(ren) guaranteed issue amount = $20,000

In addition, optional AD&D coverage can be elected for yourself or your
family.


Optional AD&D insurance is available in the following increments:

X Single—$100,000, $175,000, or $250,000

X Family—$100,000, $175,000, or $250,000

Disability Insurance Coverage


The goal of the F+W’s disability insurance plans is to provide you with
income replacement should you become disabled and unable to work due
to a non-work-related illness or injury. The short term disability plan is
voluntary and 100 percent employee paid. The long term disability plan is
paid for by F+W and offered through AIG.
