Who is Eligible
to Enroll For
Benefits?
Employees
Regular full-time or permanent part-time employees
who are regularly scheduled to work at least 24 hours
per week.
Dependents
Dependents eligible for coverage under the beneit
plans include your legal spouse* and your child(ren)
under the age of 26. The deinition of “child” includes
any of the following:
Your birth child
Your stepchild
Your legally adopted child
A child placed with you for adoption
A child for whom you or your spouse have
legal guardianship
A child for whom you are required to provide
healthcare coverage under a Qualiied Medical
Child Support Order (QMCSO) or other court
administrative order
Your eligible children of any age who became
handicapped or totally disabled before age 26, as
long as they were covered by a CommScope health
plan since their 26th birthday.
* Note: Your legal spouse is not eligible for medical
coverage if he or she is employed by another employer
and eligible for that employer’s medical coverage.
