trust for the payment of benefits under this Plan, or to do neither and pay benefits under this
Plan from its general assets, either as mandated by law or as the Employer deems advisable. In
addition, the Employer shall have the right to alter, modify, or terminate any method or methods
used to fund the payment of benefits under this Plan, including, but not limited to, any trust or
insurance policy.
If any benefit is funded by the purchase of insurance, the benefit shall be payable solely by the
Insurer.
Insurers/Claims Administrators
For fully-insured Benefit Programs, the Insurer is responsible for administering benefits and
paying claims. They may contract with a separate Claims Administrator to process claims. You
may contact the Insurer/Claims Administrator directly, using the information listed below.
While these service providers make every attempt to provide accurate information, mistakes can
occur. It is important to understand that Federal law requires that the Plan Documents always
control, even if their terms conflict with information given to you by an Insurer or other service
provider.
