 Open Enrollment is from October 15 – October 20 . You MUST

make your elections by October 20 .
 Discuss the Plan(s) with your family and determine what Plan(s)
fits your needs.

 Be sure to complete a NEW Beneficiary Designation Form for
the Group Life Insurance provided to you. It is a good idea to
update this every year during Open Enrollment or should you
experience a Qualifying Life Event to make ensure the person

you wish to receive your benefits is on file.

 If you choose to increase or newly enroll in the Voluntary Life
Insurance Plan, you MUST complete an Evidence of Insurability
(EOI) Form regardless of the amount. Failure to provide the
form will result in your request being denied by Mutual of
Omaha.

 Please notify your physicians and pharmacy of the changes
to your insurance plan and provide them a copy of your
insurance card to guarantee claims are submitted correctly.
