Short Term Disability (STD) Coverage

To help protect your financial stability in the event you became
ill or disabled, Allied Printing provides a salary continuation plan
through a short-term disability program also through LFG.

Short Term Disability (STD) provides income protection in the
DRAFT
event you become temporarily disabled due to a non-work
related illness or injury. (If you remain disabled when your STD
benefits end, you may be eligible for LTD benefits which are
outline on the following page.)

This is a contributory plan and you are required to pay a portion
of the premium. Your STD benefits are outlines as follows:


STD Benefit

Weekly Benefit 60% of Pre-Disability Earnings

Minimum/Maximum Weekly Benefit 10% / $1,000
7 days for accident
Elimination Period
7 days for sickness
Benefit Duration 13 weeks Duration

Pre-Existing Condition Limitation None



Long Term Disability (LTD) Coverage

Long Term Disability (LTD) provides income to employees who are
disabled for an extended period of time. The LTD plan from LFG is
designed to help protect your income in the event that you are unable
to work due to a covered disability.

Your coverage effective date or any increase in coverage may be
delayed if you are disabled on the date coverage is scheduled to take
effect. Review the Summary Plan Description for details.
